“Everyone in this room is on Epic’s roadmap” Verbatim quote from one of the vendors I caught up with at HIMSS this week. While maybe a slight exaggeration, it’s not far off. After years of new point solutions saturating the market, it’s clear that the pendulum has swung back to a preference for bundled products amongst health systems and large provider organizations. For vendors, the fear of Epic building a "good enough" product to prevent buyers from considering another solution is real. If your product is likely to be somewhere on Epic’s roadmap, you have a couple of options: 1) Create a product that is so good you can overcome the buyer’s organizational inertia that leads them to go with the native Epic solution. or 2) Stay ahead of Epic by continuing to add new products and use cases that they haven’t tackled yet, ideally in such a way that you can build a “platform” solution that credibly sits alongside it. (Or a combination of both 1 and 2) Will be interesting to see these dynamics play out over the next couple of years, especially in the realm of gen AI driven solutions. Epic has been public about the many use cases they’re going after, but how quickly they’re able to move, as well as how good the solutions are relative to the many other products coming to market will be fascinating to watch 🍿 Curious to hear what others think.

One of our hospitals is about to begin an Epic rollout. Please keep me in your thoughts during this difficult time. I'm no stranger to Epic -- been using it at another hospital for years. Epic updates and implementations are rapidly replacing Joint Commission visits as the most anxiety inducing hospital events. Epic as a brand has become synonymous with healthcare, on par with Tylenol, Band-Aid, and Ozempic. Epic and its EMR remind me of 90's era Microsoft and Windows. Companies using their market position to push a ubiquitous operating system that many find convoluted, cumbersome, and user unfriendly. Updates that add features no one asked for, bring unnecessary changes, and fix things that aren't broken. Extremely high switching costs that ensure continued dominance. MSFT and CEO Bill Gates were eventually called to the carpet by the DOJ -- a case that threatened to break up the company but was eventually settled. Particle Health is now suing Epic claiming monopoly status. The parallels between Epic EMR and MSFT Windows make for an interesting legal case study (alas, I'm no lawyer). To be fair, Epic isn't completely to blame. In many ways, mandated policies and procedures helped create the monster and continue to feed it. Medical records are supposed to be a repository of high value information facilitating communication and crafting a clear picture of a patient's history. Instead, they're lowered the barrier to over-protocolization, check-box policies that take away from patient care, and death by 1,000 clicks. Epic didn't create these problems, but it did facilitate them. It's not all bad with EMRs. We can't live in the past of paper charts (as efficient as they may be). But we are not leveraging technology to its best potential. What we have is a co-dependent relationship of technology and policy gone wild. MSFT ended up with a slap on the wrist, but Windows is no longer its flagship product. A post-PC era, ushered in by tablets and cellphones, changed the market. Will something similar happen to Epic and EMRs? Maybe. Excellent read on a complex topic, well researched and thoughtfully written. Articulation of a situation that has been slowly evolving and shaping in the industry for 2 decades…to land us where we are today. I’ve personally experienced first hand the “protective moats”, “chilling effect” and “straight jacket” of trying to innovate, compete, and interoperate in “an Epic world”. It ain’t easy… Is the current state of Epic’s stronghold akin to DOJ’s recent antitrust investigation of UnitedHealthcare/Optum, where potentially a ‘tipping point’ has finally been reached? Or comparable to the US vs Microsoft suit in the late 90s? (we know how that ended) Or, rather, is this instead the result of brilliant unique differentiation (Acute + Ambulatory) combined with superbly executed long-game business strategy and relentless customer-first innovation? (Ok, perhaps supported via favorable market forces too) Somewhere in between?   Free-market social-capitalist economic gone awry? Worth the read…

Under evaluation considerations add maturity of cybersecurity performance goals for EHRs and the increasing number of third party systems that integrate with your EHR (both internal and external third party systems). Health systems need to monitor the cybersecurity risk across their integrated ecosystem. The risk to patient safety and increased security costs with third party systems likely influence decisions toward a simpler integrated ecosystem of products with EHR as the base. 2. Our EHR/clinical ecosystem and the technology requirements for success mirrors an ever increasingly complex healthcare system where entry to innovation is likely difficult. What can said about a simplified healthcare delivery system?
