In a recent American Journal of Emergency Medicine article, results of an emergency department-based program that provided screening of health-related social needs (HRSN) and subsequent navigation support for high-risk patients (high ED utilizers) were discussed. The HRSN assessment and navigation resulted in a 10% reduction in ED utilization in the subsequent 6 months, even with only one third of identified HRSN being successfully resolved. Collaborative working means becoming greater than the sum of your parts â with real benefits for both patients and providers. But what does that really look like in reality? In a new blog, Dr Sian Stanley discusses how her GP practice worked with community and acute partners and others to set up an urgent treatment centre in Harlow. The project was in response to the unsustainable pressures faced by A&E, and to date, the UTC has started to reduce some pressure on the local emergency department. In particular, fewer diagnostic procedures are now taking plan in A&E, equating to cost savings for the ICB. More joined up working is also making a difference for patients, with better triaging and more use of alternative care options, which are re-directing people away from A&E and enabling them to get care closer to home. This means saving time and avoiding risks associated with being admitted to hospital, particularly for older people. VIDEO. I watch a number of videos of various Winnipeg, Manitoba, hospitals, and I learn about healthcare approaches in the province. How over the 2 past centuries the merger of 3 hospitals into Winnipeg's biggest hospital, the Winnipeg Health Science Centre, HSC for short. The Women's hospital, completely in glass so that there is a feeling of connection to the outside and the community. The biggest palliative centre in another hospital, the fireworks displays on New Year's by the Riverside Hospital, with citizens coming to see it and causing traffic lines. It is very interesting learning about hospitals across Canada in the different provinces and the different approaches and philosophies that each hospital has which is represented in the design of the hospital.

With this month marking the Universal Care Planâs (UCP) second anniversary, Tomas Ince, UCP Senior Clinical Transformation Manager, has shared valuable insights on the milestones over the past two years and the benefits of the integrated approach that the Universal Care Plan brings to the people of London. With key insights into what to look forward to for the future. ð Having supported Better for the past three years with the integration of the Universal Care Plan for OneLondon, ReStart are extremely proud to be part of a project which is having such an impact and helping to improve patient care. As commented by Tom Ince; âAs of June 2024 over 64,000 Londoners had an active UCP, with over 16,000 being created since the beginning of the year. That is the most since the UCPâs conception and the growth over the past six months has been a joy to behold.â In terms of future developments Tom goes on to explain: "We have had some great achievements in recent months, but our digital journey is never complete. EM plays an important role in supporting the management of these events whether itâs coordinating essential resources, working with leaders and clinical staff to find solutions for optimizing flow, and implementing contingency plans or alternate care spaces which can ensure effective response and patient care. This expertise and the ability to convene interdepartmental teams helps to mitigate the impact of overcrowding and maintains the overall functioning of health care facilities. The horizontally and vertically integrated continuum of EMs at the facility/system, local, regional, and state government levels works to ensure care can still be provided appropriately but, as with anything in this world, itâs not without distinct challenges. ð«
