Partnership in Care: Best Practices for Hospice Provider and Facility Partner Collaboration 📘 Providing patient-centered, end-of-life care to residents of facilities is an important aspect of many hospice organizations’ services that can differ broadly from providing care in the home setting. Developed by a combined workgroup of Regulatory and Quality and Standards Committee members, Partnership in Care: Best Practices for Hospice Provider and Facility Partner Collaboration is a fillable tool to facilitate collaboration between hospice and facility staff, and to provide a framework to guide provider conversations between hospices and facility partners to establish best practices in caring for shared patients.     The Centers for Medicare & Medicaid Services (CMS) plans to carry out the hospice Special Focus Program (SFP) based on bad data. If implemented with its current flaws, this program threatens patients and families’ ability to make informed decisions about the best hospice care. The bipartisan Enhancing Hospice Oversight and Transparency Act (H.R. 10097) will ensure that CMS has the time and tools needed to fix this flawed program.     We Honor Veterans Partners with Axxess to Expand Provider Education for Veterans 🔴 ⚪ 🔵 The Alliance’s We Honor Veterans (WHV) program has partnered with Axxess to provide prospective and current WHV partners with education that introduces them to or deepens their understanding of We Honor Veterans. In honor of Veterans Day, Axxess has launched the program with a course focused on providers new to the WHV program. The full Axxess training library will include courses tailored to care-at-home executives and current partners, in addition to the existing new partner course. Upon completing the training, partners will be equipped to continuously improve the care they provide to the Veteran community and successfully navigate the robust library of provider education available through the We Honor Veterans website. Together, these resources offer best practices and guidance for caring for Veterans at the end of life for organizations new to or at any level of the We Honor Veterans program.   • This week is Hospice Week! 👏 Hospice care, and the Medicare hospice benefit, is one of the most sacred, successful, and high value parts of the United States healthcare system and the Medicare program overall. This week we recognize the many care team members nationwide who make this work possible, and we celebrate the demonstrated value of hospice care for patients and families and for Medicare. Hospice care prioritizes comfort and quality of life by reducing pain and suffering. Although some hospice care is provided in hospitals, nursing homes, or other facilities, most patients are cared for in the place they call home, which is where most people would prefer to be. Hospice provides all medications, services, and equipment necessary. Hospice also offers bereavement care (grief support) for up to 13 months.
