Additionally, consider the frequency of similar occurrences. Repeated near-misses in the same area or involving the same process may indicate a systemic issue that needs immediate attention. By focusing on high-risk incidents and patterns, you can address the root causes more effectively and prevent future accidents. This targeted approach not only enhances workplace safety but also maximises resources by addressing the most critical risks first.

We use a Near Miss Classification Matrix to evaluate incidents based on their nature and potential consequences, assigning a score from 1 to 20. Prioritization is determined by the risk matrix:
High Priority: Scores of 15-20
Medium Priority: Scores of 5-14
Low Priority: Scores of 1-4
Once prioritized, we follow a nine-step incident investigation process to identify root causes, system gaps, and develop recommendations for implementation.

Immediate attention should be given to incidents that pose ongoing risks to personnel, assets, or operations to prevent future accidents.
