I have found that communication is key. Excellent communication between team members is achieved through periodic follow up meetings with the participation of members from all the areas involved, regardless the phase the project is in. These meetings are set to discus the next milestone by key players from all the areas involved in the project. This way everyone knows when they enter and leave, when they pass the baton and how they contribute to the achievement of the objectives and success of the project.

One thing I've found helpful in aligning teams, is having a kick off meeting. In the kick off meeting for the project, ensure that 1 member from each team is present. Establish clear accountability lines, Establish check in frequency, ideally weekly, as well as the structure for each meeting. Keeping them succinct and to the point. Have a process for handling any difficulties, whether all members work in them together, or if only some members are needed, how they'll meet and report back. Have a separate person keep the minutes, and make sure you have an agenda, and someone to keep timing. Hold each other accountable to the schedule, and to the items to be completed.
