A key part of the issue management process is hazard analysis - how big will an issue become rather than how big it is today. A low occurrence issue today could turn into a large failure rate if the risk is not fully understood. Severity of the issue should also be used as a multiplier of risk alongside the failure rate - low occurrence high severity issues are a must fix in the same way a high occurrence low severity issue is. This also allows you to decide on which issues you do not work on - low occurrence low severity can be deprioritised or not fixed at all depending on capacity.

In my experience, documentation and use proper communication platforms are valid part of project/programme management. - Selected platforms for communication and documentation should be suitable for project/programme needs. Also project teams skills should be evaluated carefully. If you have a inexperienced team, trying to use complex platforms like Jira or Confluence will fail and create push back from project/programme team members. Selected platforms should be suitable with project needs also. If the project is non-complex, try to avoid highly capable platforms and keep all information/communication tools simple like Ms Planner/ Ms Teams.
