Atendia um senhor de idade que precisava resolver sua questão financeira. Ele não possuia nenhum entendimento sobre como resolver esse problema e eu fiquei horas até que ele enfim entendesse o que precisava ser feito. If it’s possible, guide your client to do the application/case management task, don’t do it for them. The point of case management is, yes, get benefits of some kind, but also teach the client how to retain those benefits and empower them to be independent.

To assess the needs and preferences of a client using a client-centered approach, engage in open-ended conversations, actively listen, and ask open-ended questions to understand their unique situation. Tailor assessments to the individual, considering their values, cultural background, and personal goals. Regularly check in to ensure their evolving needs are met, fostering a collaborative and adaptable approach.

The Care Plan MUST be created with the client’s goals using words and methods the client understands. It’s difficult to make sure we are engaged with the client and not leading them to set goals that are unattainable or not what they want.

Implement the care plan by following its details, document activities, and communicate effectively. Monitor the client's progress, regularly review the care plan, and adjust as needed. Provide training and education to the client's support network.

A critical component of the discharge or transition plan which is overlooked by many providers is a continuing care phase post discharge. The purpose of this phase is to determine if the client has connected with the referrals and to provide an open door if problems arise or relapse occurs. It also provides staff with data and information as to what clients are doing to maintain stability or sobriety.
