Without knowing actual reason for quality issues we can't improve. 2) After tracing out the root cause, an effective corrective action is required to stop such instances to repeat. 3) There should be a collaborative approach to have a good communication. 4) Look out for further gaps in system and close them to avoid such incidents to happen in future. 5) Revise your process and document the changes that you implemented.
