Either you want each user to have at least one album or not. This is what IX-D9 does.
If you do not want them to show up if they are not used, i.c. empty, Tick Table II-E7. However, the admin and the album owner will see them – even empty – to be able to upload photos to them at the front-end.
If you want to get rid of empty albums, run Table VIII-B7.
A situation where it auto creates more than one album can only be reproduced if you defined more than one grant parent. In this case all grant parents will have an album owned by any user that ever logged in after these settings became effective.

After installation I followed the directions of your site “How to start with Photo Album Plus”.
I have made no special configuration, especially for grandparents..
All albums are created with my name on each connection with the following values.
Wordpress 4.6.1 with Vantage theme

I tested it on two different dev sites and i still can not reproduce this problem.
Do the following: Run Table VIII-A1: Setup. This fixes possible incomplete initialisations.
In Tabe IX-D, verify the settings 9 and 11. If they are correct, change them anyway and back to what they should be.
