I had a similar issue:
The template contained a background image and my company logo. When the PDF was created the background image was missing.
Solution: I removed the logo and then the background image was shown in the PDF.
So I just added the logo to the background image an the PDF looks fine, now.

I have tried removing logo too but unfortunately it is not working for me. PDF get created but there is no background image. Please share if somebody found solution. Thank you
