Not generating or attaching PDF

    Currently at Version 4.0.1.

    After submitting the form, the response is received via email, but no PDF is attached. We have configured both MAIL and MAIL(2), but neither has a PDF attached. This just recently started happening.

    Please try these steps for us:
    Deactivate All Plugins Installed on Your Website
    You can then reactivate these plugins one by one, to see when the error happens again.
    Switch to a Default WordPress Theme and try to troubleshoot
