    AFter updating to WordPress 5.5., your plugin stoped passing the variable to my form. I installed plugin Enable jQuery Migrater and that fixed the issue and the site is working again. But I believe your plugin may be using old jQuery calls so your plugin may need to be updated.

    There is no javascript/jQuery at all in this plugin.

    Sounds like you may have had some other JS issue on your site that was breaking things in some way, but all the DTX does is take the output of a shortcode and place it as the default text in an input field on the server-side. There is no JS involved at all.
